NYC School Survey: Your School. Your Child. Your Voice.

What is the NYC School Survey?

Every year, all parents and teachers in grades 3K-12 and select school support staff take the NYC School Survey. The survey helps school leaders understand what key members of their school community think about the learning environment at their school. The information captured by the survey is designed to support a dialogue among all members of the school community about how to make the school a better place to learn.

How to Take The Survey

Parents/Guardians:

Enter your access code:  the letter f, followed by your child’s nine-digit Student Identification (OSIS) Number to access the survey. Note that “ f ” must be lowercase. Families can find their child’s Student Identification Number on their report card, student ID card, or New York City Schools Account (NYCSA).

P.S. 215 Teachers/Support Staff:

If you did not receive an access code, contact your school survey coordinator, Vincenza Gonsalves, to see if you are eligible and receive your code.

Survey Ends June 18
